Vision Provider Lookup

The link below will take you to the vision provider lookup. On the page that follows after clicking the link:

1. If you are not currently an AmeriValuePlan member:

  • Select "No" to to question "Are you a VSP member?" (AmeriValuePlan members are automatically VSP members).
  • On the "Not a Member?" pop-up that appears after selecting "No", click on "Continue without signing on".
  • Some random numbers and letters will appear. Enter the sequence in the text box below it. Proceed to Step 2.

2. To query the provider list:

  • Scroll down to the section marked "Step 2"
  • Enter your search criteria in either the "Address Search", "Name Search", or "Office Name Search".
  • Click the "Submit" button that corresponds to the section where you entered your data.
